Medicare

We accept Medicare Part B insurance and some Medicare Advantage PPO plans with OON benefits.

Other Insurances

We are OON (out of network ) with all other insurances. In this instance, you will pay for treatment at time of service, and we can provide you with a superbill which you can submit to your insurance company for possible reimbursement.

Private Pay Rates:

Physical Therapy or Wellness Evaluation: $150

Physical Therapy or Wellness Session: $120

Pre-paid 10-session package: $1,050

LSVT BIG Parkinson’s Program (eval plus 4 consecutive visits/week x 4 weeks = 17 visits): $1,700

Payment:

All payments are due at time of service.

Good Faith Estimate (for those not using insurance)

Notice: ​You have the right to receive a “Good Faith Estimate” (GFE) explaining how much your medical care will cost

​Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

•This new regulation is designed to provide transparency to patients regarding their expected medical expenses and to protect them from surprises when they receive their medical bills. It allows patients to understand how much their health care will cost before they receive services.
• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
• Make sure to save a copy or picture of your Good Faith Estimate.

There are a number of factors that make it challenging to provide an estimate on how long it will take for a client to complete therapy services, and much depends on the individual client and their goals in seeking therapy. Treatment strategies and frequencies are constantly being reassessed in order to provide the client with the highest likelihood of meeting their goals. Ultimately, as the client, it is your decision whether to continue with the recommended treatment, decrease frequency or to stop therapy.

When we first make contact either via email or phone, we will verbally provide you with a Good Faith Estimate (GFE). In addition, this estimate will be made available to you in writing. If you are a current client, we will be providing you with a GFE as well.
